Please visit greenwaveathletics.org and follow the “more” tab to access Final Forms. Select your activities for the upcoming season. Athletes and parents need to complete ALL of the digital signatures in order to participate.

Pre-Participation Physical

All athletes at Holy Name will need to have a current physical on file with the HN Athletic Office prior to practicing or competing. These can be downloaded from greenwaveathletics.org under the “more” tab and dropped off at the athletic office.
